This week, the Community Care HUB, an initiative of the Center for Population Health (CPH), celebrated its 5th anniversary.

“Over the past five years, the Community Care HUB has shown the power of collaboration in action. By working hand-in-hand with our community partners and dedicated Community Health Workers, we’ve been able to break down barriers to care and connect individuals and families with the support they need to thrive. This anniversary is not just a celebration of years, it’s a celebration of lives changed and a healthier future for our region, said CPH Associate Director Kimberly Roberts.

The HUB works with local agencies – Alleghenies United Cerebral Palsy, Beginnings, Inc., and the Community Help Center – who take on the role of a Care Coordination Agency (CCA). The CCAs hire and train Community Health Workers (CHWs). The CHWs work with referred at-risk individuals face-to-face, preferably in their homes, to help connect them to needed services.

The HUB model involves collaboration of all community resources to reduce both medical and social barriers to care, including employment, housing, and transportation.
