Our Story
The Center for Population Health (CPH) was developed, through funding and in collaboration with the 1889 Foundation as a population health resource center to benefit Cambria and Somerset counties. Population health is a collaborative and comprehensive approach that addresses the many things in our communities that impact health. CPH work focuses on opportunities around disease prevention, reducing health disparities, and improving the health of community members. Current strategic initiatives of the CPH include:
